WELCOME
We are pleased that you have selected our office to serve
your needs in the field of Orthopedic Surgery. It is our continuing
goal to provide you with the most comprehensive and up-to-date
health care possible. It is important that our patients become
partners with us in their own ongoing health care. Should
you have any questions regarding your treatment or office
procedures, please express them to our trained professional
staff.
YOUR FIRST VISIT
If you are a new patient, you will receive an information
form and a medical history form to complete upon signing into
our office. It is important that you bring a list of all medications
that you are presently taking. You might be asked to arrive
a little early for your scheduled appointment in order for
us to update your patient file. Please have authorization
approvals before appointment time. New patients must
bring x-rays and MRI's (if applicable) to your first office
visit.
OFFICE HOURS
Our office hours are 8:00 a.m. to 5:00 p.m. Monday through
Friday. We have someone available at all times during these
hours to assist you should the need arise.
TELEPHONE
In Fort Myers, our telephone number is 768-2272. After office
hours and on week-ends and holidays, we have a 24-hour answering
service for your safety and convenience. Our answering service
will relay your concerns to the doctor on call and he will
return your call. We request that office business, such as
appointment scheduling, billing matters, and prescription
requests be addressed during regular office hours.
APPOINTMENTS
We ask that you please realize that all patients do not require
the same amount of time to be seen by the doctor. Therefore,
a Post-op patient or a patient scheduled for a return visit
maybe be taken before a New Patient or patient requiring fracture
care. We also, occasionally, have Emergency conditions arise
which may result in minor "In Office Surgical" treatment.
